Social Support Workers
Social Support Workers at SET4CARE play a vital role in providing compassionate and personalised care to individuals in need.
To succeed in this position, candidates should have:
- A diploma, degree or postgraduate qualification in Aged Care, Nursing or a related discipline
- Relevant experience working with Aged Care and/or Disability Care
- Valid driver’s license

- Personal Care: Assisting with daily self-care tasks such as bathing, toileting, dressing, grooming and medication prompting.

Their responsibilities include:
- Monitoring patients' conditions and assessing their needs to provide personalised, high-quality care
- Observing and interpreting symptoms to effectively communicate with physicians and other medical professionals
- Collaborating with physicians and Case Managers to develop tailored care plans for patients
- Performing routine medical procedures such as blood pressure measurements, administering injections and monitoring vital signs
- Providing swift and effective medical care during emergencies
- Creating a supportive and nurturing environment that prioritises both patient and family well-being
